If clinical signs and symptoms consistent with liver disease develop, or if systemic manifestations occur, COX-2 inhibitors should be discontinued. Long-term administration of NSAIDs has resulted in renal papillary necrosis and other renal injury. Caution should be used when initiating treatment with COX-2 inhibitors in patients with considerable dehydration. It is advisable to rehydrate the patient first, and then start therapy. Caution is also recommended in patients with pre-existing kidney disease. Anemia is sometimes seen in patients receiving COX-2 inhibitors. Patients on long-term treatment should have their hemoglobin or hematocrit checked if they exhibit any signs or symptoms of anemia. Fluid retention and edema have been observed in some patients taking COX-2 inhibitors. Therefore, COX-2 inhibitors should be used with caution in patients with fluid retention, hypertension or heart failure. Patients with asthma may have aspirin-sensitive asthma. The use of aspirin in patients with aspirin sensitive asthma has been associated with severe bronchospasm which can be fatal. Since cross reactivity, including bronchospasm, between aspirin and other nonsteroidal anti-inflammatory drugs has been reported in such aspirin-sensitive patients, COX-2 inhibitors should not be administered with this form of aspirin sensitivity and should be used with caution in patients with preexisting asthma. A patient with symptoms and or signs suggesting liver dysfunction, or in whom an abnormal liver test has occurred, should be monitored carefully for evidence of the development of a more severe hepatic reaction. Use of the COX-2 drugs is not recommended in patients with severe hepatic insufficiency. Drug Interactions ACE Inhibitors Reports suggest that NSAIDs may diminish the antihypertensive effect of angiotensin converting enzyme Inhibitors. Aspirin Concomitant administration of low-dose aspirin with COX-2 drugs may result in an increased rate of GI ulceration or other complications, compared to use of COX-2 drugs alone. Furosemide Clinical studies, as well as post-marketing observations, have shown that NSAIDs can reduce the natriuretic effect of furosemide and thiazides in some patients. Fluconazole Concomitant administration of fluconazole with Bextra and Celebrex produced a significant increase in the plasma levels of valdecoxib and celecoxib. Lithium Bextra produced significant decreases in lithium serum clearance and renal clearance. Lithium serum concentrations should be monitored closely when initiating or changing therapy with Bextra. Phenytoin Patients already stabilized on Bextra should be closely monitored for loss of symptom control with phenytoin coadministration. Warfarin COX-2 inhibitors caused a statistically significant increase in plasma exposure of warfarin. Anticoagulant therapy should be monitored, particularly during the first few weeks, after initial therapy. Health economic implications of rofecoxib in the treatment of osteoarthritis. The true impact of these potential advantages on the management of patients with osteoarthritis will ultimately need to be demonstrated in clinical practice. However, because of the likely propensity for clinicians to initially reserve the use of rofecoxib for patients with a history of NSAID intolerance or at high risk of serious upper GI tract complications with NSAIDs, it may require a number of years of experience with rofecoxib before such advantages are observed. This analysis shows that in randomized clinical trials of patients with osteoarthritis, selective COX-2 inhibition with rofecoxib was associated with a significantly lower incidence of early treatment discontinuations due to GI AEs and with a lower incidence of upper GI tract symptoms relative to the comparator NSAIDs. These results suggest that inhibition of COX-1 may be at least partially responsible for the upper GI tract symptoms associated with NSAIDs and that selective COX-2 inhibition may result in greater GI tolerability than nonselective COX-1 and COX-2 inhibition. Drug Interactions ACE inhibitors Reports suggest that NSAIDs may diminish the antihypertensive effect of angiotensin converting enzyme ACE ; inhibitors. Nevertheless, during concomitant therapy with furosemide and Mobic, patients should be observed closely for signs of declining renal function, as well as to assure diuretic efficacy. Preparations. Optical signals were recorded from three in vitro preparations: the olfactory lobe of the spiny lobster Panulirus argus, using a perf used, isolated brain preparation; the lobster antennular nerve, using a cut section of nerve; and the olfactory bulb of the turtle T. carolina, using a superf used hemisected bulb preparation. The lobster brain and turtle olfactory bulb preparations have been described previously Berkowicz et al., 1994; Wachowiak and Ache, 1994 ; . In the lobster brain preparation, the animal was chilled on ice and decapitated, and the brain was cannulated via the medial artery and perf used with oxygenated Panulirus saline. The brain was then removed from the head, along with associated blood vessels, by cutting all nerve roots and the optic tracts leading to the protocerebrum. The brain was pinned in a Sylgard recording chamber with the olfactory lobes facing upward and was desheathed, and the left antennular nerve was placed in a glass suction electrode for stimulation. In the nerve preparation, a 57 mm length of nerve was excised just distal to the point where the nerve entered the brain and secured in a recording chamber containing two suction electrodes. One suction electrode was used to stimulate from one cut end, and the other electrode was used to record the compound action potential from the other end. The nerve was continuously superf used with oxygenated saline as in the isolated brain preparation. In the turtle preparation, animals were anesthetized by chilling on ice for 23 hr and decapitated, and the forebrain including the intact olfactory bulbs was removed and placed in chilled turtle Ringer's solution bubbled with 95% O2 5% C O2. The olfactory bulb from one side was isolated by making a coronal cut through the forebrain just caudal to the bulb. The bulb was then hemisected in the horizontal plane, and one hemisection was pinned down in the recording chamber with the outer surface facing upward. The olfactory nerve, which is large and distinct in turtles, was drawn into a suction electrode for stimulation. The hemisection was kept continuously superf used with turtle Ringer's solution 12 ml min ; via the same system as for the lobster, with the perf usion cannula secured in the bath. Drug deliver y and solution replacement. A three-way solenoid valve Cole-Parmer, Chicago, IL ; allowed switching between normal and C a 2 -free saline solutions in the perf usion system. Trials with a marker dye in the solution showed that complete replacement of the solution occurred at the tip of the perf usion cannula within 2 min at a flow rate of 0.5 ml min. Replacement of saline in the tissue of the lobster brain or turtle bulb was presumably slower. In the lobster the brain was perf used with C a 2 -free solution for 510 min before f urther trials, whereas in the turtle bulb 1520 min was allowed for solution replacement. Transmitter agonists and antagonists were introduced into the background perf usion via a manual four-way loop injector Rheodyne, Rohnert Park, CA ; . Injected solutions reached the cannula tip within 30 sec, and the size of the injection loop was large enough to allow a sustained peak concentration for up to 4 min. Normally, trials were performed 2 min after drug injection. The drug concentrations reported here are those injected into the perf usion system. No attempt was made to correct for possible dilution in the brain or in the bath. Optical recordings. For the voltage-sensitive dye experiments, lobster brain tissue was stained with di-2-AN EPHQ JPW-2081; synthesized by J. P. Wuskell and L. M. L oew, University of Connecticut Health C enter, Farmington, C T ; by introducing 2 ml dye 0.06 mg ml ; into the perf usion line or by applying dye 0.36 mg ml ; in the bath for 20 min. Dye perf usion resulted in uniform staining of the brain, whereas bath application stained only the outermost 50 100 m of tissue. T urtle olfactory bulb tissue was stained by bath application of di-4-AN EPPS 0.05 mg ml ; or RH-414 0.05 mg ml ; Grinvald et al., 1994 ; both from Molecular Probes, Eugene, OR ; for 30 min. For the turtle calcium-dye experiments, olfactory receptor axon terminals were selectively labeled by anterograde transport of the dextranconjugated calcium indicator C alcium Green-1 or C alcium Crimson both 10 kDa; Molecular Probes ; applied to the olfactory epithelium using a protocol adapted from Friedrich and Korsching 1997 ; . T urtles were first chilled on ice for 12 hr, then placed upside down with the mouth held open; 25 60 l of dye solution dissolved in 0.1 M NaC l plus 0.5% Triton X-100 was injected into each naris. The pharyngeal opening of each naris was plugged to prevent leakage, and the epithelium was stained for 10 20 min, after which the plugs were removed and the mouth closed, allowing the animal to expel the dye from the nose.
