To explore the feasibility and effectiveness of brief interventions to prevent depression in older subjects, we searched 3 databases for potentially relevant clinical trials; the bibliographies of relevant trials were searched for additional references. Nine studies met the following five inclusion criteria: original research, subjects mean age 50 years or more, controlled trial of a brief 12 weeks ; intervention to prevent depression, determination of depression status 12 months or more after enrolment, use of an acceptable depression outcome measure. As to feasibility, study enrolment rates were 21 to 100% median 45% completion rates were 53% to 100% median 76.5% compliance rates were 29% to 100% median 93.5% ; . As to effectiveness, 4 trials had positive results. In 2, there were statistically significant differences in depression symptom outcome scores favoring the intervention group; in 3, absolute risk reductions were -17% to 45% median 2.3% relative risk reductions were -125% to 71% median 45% ; . Although the evidence is limited, trials of feasible and effective interventions were characterized by enrolment of subjects at high risk of depression and the use of education and cognitivebehavioural techniques to empower and reduce negative thinking.
