Imuran elevated liver enzymesTwo to four percentages of allergic reactions. The allergic reaction can be mild, which is a little bit of a rash. Or they could be more severe with a chest and difficulty breathing, but they're all treatable. Infliximab has to, any antibody, any biologicals, you can't take it by mouth necessarily because the stomach and intestine would break it down and digest it and it wouldn't get in to the bloodstream where it has to have its action so all these biologicals. One downside is that it has to be taken either as an IV infusion into the vein or as an injection. Many of us are somewhat needlephobic but these are very effective drugs. But, even in the face of using them in patients who have more significant inflammation going on, it probably reaches a 70 to percent effective rate. There are some other things that we do with it. We sometimes increase levels of infusion. We give them in shorter intervals, and they can be very effective. The problem is that the body can build up a tolerance to them, and we don't always like to give them routinely unless we've hit certain obstacles in treatment. But they've spared many children the need to have steroids. They've also eliminated the need for hospitalization sometimes because they're very quickacting. Rick: And over time, do these drugs actually prevent relapses or disease progression? Dr. Benkov: Well we're still not - there's a little bit of a debate among the medical community whether they should be used on an ongoing basis or whether they should just be used for tune-ups. The medication we like to use for an ongoing basis are some of the more standard traditional ones that can be taken orally, such as 6mercaptopurine or azathioprine, which is also called Imuran. These drugs seem very safe and tolerated over a period of time. We use infliximab or Remicade basically to bridge the time that these other medications will need to take effect. What do you know about Antegren in terms of how the clinical trials have gone, how promising have they been, and how do you anticipate it being accepted? Dr. Benkov: Well, Antegren again is not quite yet released, but it should be coming out very shortly. The trial seems as promising as infliximab. We ha ve little bit of room in pediatrics because these drugs usually aren't released for children. What typically happens is they're approved for adults and once they come out in adults, we slowly come around to using them. We probably use infliximab as much in children as in adults.
