Nimodipine 1.25-5 mg kg IP ; and nitrendipine 1.25-5 mg kg IP ; also significantly increased the yawning response. The DHP calcium channel blockers alone induced only a low incidence of yawning. The effects of nifedipine on physostigmine-induced yawning were reversed by the DHP calcium channel activator BAY K 8644 which also inhibited yawning induced by physostigmine 100 micrograms kg SC ; and pilocarpine 2 mg kg SC ; . In contrast to the DHP compounds, diltiazem 2.5-10 mg kg IP ; and verapamil 2.5-10 mg kg IP ; failed to potentiate yawning. Sulpiride 10 mg kg SC ; antagonised the nifedipine potentiation of apomorphine-induced yawning but not that of physostigmine-induced yawning; atropine 2.5 mg kg SC ; antagonised both effects. These results support the hypothesis that this effect of dihydropyridine compounds is not dependent on, nor mediated through, dopaminergic mechanisms. Bowron, A. 2004 ; . Average healing rates increased from 68% at four weeks to 84% by eight weeks, but there was significant heterogeneity between the studies.1 A meta-analysis of 10 RCTs n 10, 176 ; conducted for the NICE guideline compared double-dose with full-dose PPI and found that doubling the dose of a PPI only has a small effect on healing rates for oesophagitis at four weeks.1 The average healing rate was increased from 72% to 77% NNT 19 ; . Post-hoc sub-group analysis of two studies suggests that larger gains may be seen in patients with severe oesophagitis Los Angeles grade C or D ; Therefore, double doses are only appropriate if there is endoscopic evidence of severe oesophagitis.1 It is estimated that 6080% of patients with successfully treated GORD will symptomatically relapse within one year if they are not provided with maintenance therapy.1 However, this also means that 2040% can be returned to self-care and may not require prescribed medication. Evidence currently suggests that the most effective therapy to prevent relapse is a fulldose PPI, the next best is a low-dose PPI, then an H2RA. NICE analyses found that, over 6- to 12-month follow-up, full-dose PPIs: 1 reduced the rate of relapse from 79% to 24% when compared with placebo NNT 2 ; were more effective than low dose PPIs relapse rate 15% vs. 28%, NNT 8 ; were more effective than H2RA therapy relapse rate 20% vs. 59%, NNT 3 ; . However, NICE advises that, if symptoms recur following initial treatment of GORD, a PPI should be offered at the lowest dose possible to control symptoms, on an as required basis.9 H2RA or prokinetic therapy may also be considered as alternatives if there is an inadequate response to a PPI, since individual patients may respond to them and cromolyn.

A published randomised controlled trial compared the Yuzpe regimen with the levonorgestrel regimen two doses of 750mcg 12 hours apart ; in 834 women within 48 hours of coitus.2 The failure rate was similar with both regimens 2.6% with Yuzpe and 2.4% levonorgestrel respectively ; . The two regimens were again compared in a WHO multi-centre study in 1998 women.3 This was a well conducted study with a very small loss to followup rate of 2.2%. The levonorgestrel regimen was significantly more effective and caused significantly less nausea and vomiting. Other reported side effects with levonorgestrel include breast tenderness, headache, dizziness and fatigue. There may be a temporary disturbance of bleeding patterns but most women will have their next menstrual period at the expected time. Some women may experience bleeding or spotting after treatment. The only absolute contraindications to use of Levonelle-2 are unexplained vaginal bleeding, current breast cancer, pregnancy or hypersensitivity to any ingredient. As it contains no oestrogen, it may be the hormonal method of choice in women with a history of venous thrombosis.

DETTMAR2, 1 Nutrition Research Centre, London South Bank University, London, UK, SE1 0AA and 2Technostics, The Deep Business Centre, Tower Street, Hull, East Yorkshire, UK, HU1 4BG Previous studies have determined that diet and lifestyle factors can contribute to the onset of symptoms of gastro-oesophageal reflux disease1, 2. It is uncertain which factors are the most influential to the onset of heartburn as an independent gastrointestinal disorder3. Therefore there is a need to define these factors in order to minimise the risk of progression of heartburn to gastro-oesophageal reflux disease, Barrett's oesophagus and oesophageal adenocarcincoma4. The aim of the present study was to investigate the perceived symptom trigger factors from individuals suffering from heartburn. Fifteen Caucasian volunteers, 10 females of mean age 51.7 years age range 37-62 ; and 5 males of mean age 57.6 years age range 41-64 ; , participated in focus group interviews at The Luton and Dunstable Hospital NHS Trust. Subjects were recruited from poster advertisements displayed throughout the hospital and from a local general practitioner clinic in Luton, Bedfordshire. Subjects were screened with the use of a questionnaire and those subjects who presented with frequent heartburn were invited to participate. Ethical approval was gained from the Bedfordshire Local Research and Ethics Committee. Multiple factors were perceived by heartburn sufferers as triggering their symptoms. The key dietary factors reported by participants included eating late 46% ; , the consumption of: bread 33% fatty foods 33% spicy foods 33% large meals 33% ; and the high temperature of food and beverages 33% ; . The key lifestyle factors reported by participants included alcohol 60% posture 60% weight gain 46% ; and stress 40% ; . The findings of this study identify that there are several symptom triggers for heartburn as an independent gastrointestinal disorder, which are in accordance with those previously reported for gastro-oesophageal reflux disease as a whole5. This suggests that these diet and lifestyle factors may be contributory to the progression of heartburn to gastro-oesophageal reflux disease. In addition to these findings new symptom triggers for heartburn have come to light: consumption of bread and hot temperature of food and beverages. The findings for the consumption of bread are in contrast to Nilsson et al, 2004; El-Serag et al, 2005 who reported that due to its high fibre content bread may reduce the risk of gastro-oesophageal reflux disease1, 2. The consumption of food and beverages at a high temperature has only previously been associated with the risk of oesophageal cancer6. Further work is needed to find out if these perceived trigger factors are a reality, by using a specifically designed symptoms and triggers diary focussing on both diet and lifestyle in heartburn patients and ddavp.
